About this tool

Add fields, give each a label, and the form builds below with a live preview. The name sent to the server follows the label until you change it yourself. Every field gets a proper <label for> linked to its id, so clicking the label works and screen readers announce it; hint text is linked with aria-describedby, and radio buttons are grouped in a <fieldset> with a <legend>. Use the arrows to reorder fields.

The markup includes the details hand-written forms often miss: autocomplete values for names, email, phone and passwords so browsers can fill them in, a “Choose…” first option on dropdowns so a required choice can't be skipped by accident, unique ids even when names repeat, and a note explaining the asterisk on required fields. Nothing is submitted anywhere; the preview is sandboxed and its button does nothing. You'll need a server script or form service at the action URL to receive the data.
