About this tool

A good review looks back and forward. For each objective set last time, record what actually happened and a rating if you use them. Then note strengths with evidence, one or two areas to develop (not a long list), and goals for the next period. Writing goals the SMART way, specific, measurable, achievable, relevant and time-bound, makes the next review much easier: “mentor one new starter through their first three months by March” rather than “help more with training”.

Reviews should be a conversation, so leave room for the employee’s own comments and agree the support they need, whether training, coaching or time. Nothing in the written summary should come as a surprise to them: raise concerns during the year, not for the first time in the review. Copy the form into Word, print it for signing or download it. Your notes are remembered on this device.
